We Build Hope One Brick at a Time

The mission of SSM Health Cancer Care is to nurture, and provide hope and strength to patients and their families.

This program is designed to honor or remember loved ones who struggled with cancer. A gift of $150 or more will recognize that person on an engraved plaque on the Wall of Hope, located along the outside entrance to the SSM Health Cancer Center’s building. The plaque can be inscribed as the donor chooses and helps offer the latest technology and services to patients.

To donate, please download the Cancer Care Fund donation form and make checks payable to the Agnesian HealthCare Foundation. Return checks to 430 E. Division Street, Fond du Lac, WI 54935.
