Established in 2000, the SSM Health St. Clare Health Foundation is a not-for-profit corporation under the laws of the State of Wisconsin and a tax-exempt organization under section 501(c)(3) of the Internal Revenue Code.

Gifts to the SSM Health St. Clare Foundation - Baraboo improve the quality of health care services in the greater Baraboo, Lake Delton, Wisconsin Dells area. Financial support from the community makes it possible for the Foundation to purchase vital, new pieces of medical equipment, add new health care facilities, assist those who are uninsured, and develop new health care and educational programs, services and initiatives. Memorials & Tributes

Celebrate a milestone, honor someone special, or remember a loved one.

All gifts to SSM Health St. Clare Foundation can be made ‘In Memory’ or ‘In Honor’ of someone special. Your gift can be designated to the fund of your choice, and you may provide the name and address of a person or family you wish to be informed of your tribute. Gift amounts are never shared.

Funeral Memorials

If you have recently lost a loved one and would like to direct memorials to SSM Health St. Clare Hospital, it’s important that you include your wishes in the obituary notice. Wording most often used is, “The family asks that memorial gifts be made to SSM Health St. Clare Foundation, 707 Fourteenth St., Baraboo, WI 53913 or visit givetossmhealth.org/DonateBaraboo.”
