SSM Health at Home Foundation of Wisconsin

The SSM Health at Home Foundation of Wisconsin is a nonprofit organization that supports the programs and initiatives of SSM Health at Home in Wisconsin. SSM Health at Home provides assistance across southern Wisconsin to those facing health related issues. When you make a donation, you are helping people living in your community who are recovering from serious illness or injury, families coping with terminal illness and grief, seniors dealing with hunger, and your neighbors in need through our community programs.

Donations to home care or hospice in the Greater Fond du Lac or Monroe areas can be made through the Agnesian HealthCare Foundation or the Monroe Clinic & Hospital Foundation.
