Ways to Support

Thank you for considering a gift to the Agnesian HealthCare Foundation.

Aside from major capital campaigns, Greatest Need funding has the greatest impact on our patients, families, and staff each year. These gifts are vital to the work of the Foundation and provide the most flexibility to meet the emerging needs of our Hospitals and the communities we serve.

Gifts can be made in a variety of ways:

Types of Gifts

Memorials & Tributes

Celebrate a milestone, honor someone special, or remember a loved one.

All gifts to the Agnesian HealthCare Foundation can be made ‘In Memory’ or ‘In Honor’ of someone special. Your gift can be designated to the fund of your choice, and you may provide the name and address of a person or family you wish to be informed of your tribute. Gift amounts are never shared.

Funeral Memorials

If you have recently lost a loved one and would like to direct memorials to the Agnesian HealthCare Foundation, it’s important that you include your wishes in the obituary notice. Wording most often used is, “The family asks that memorial gifts be made to the Agnesian HealthCare Foundation, 430 E. Division Street, Fond du Lac, Wisconsin 54935 or visit ssmhealth.com/givingfdl."

Endowments

A memorial or tribute gift can be established in the form of a named endowment with a minimum gift of $25,000 to fund a specific interest that is consistent with the mission and goals of SSM Health in the Greater Fond du Lac area.

With an endowment, the principal is invested to create a source of income. Only the earnings are used to support the purpose for which the endowment was established.

Your tribute gift will provide a lasting memory of the honored person and will benefit your chosen cause in perpetuity. Contact the Foundation to learn more about endowments.

Planned Giving

When you give a planned gift for the future, you are leaving a legacy – a reflection of your values, priorities and commitment to support exceptional health care in your community. We encourage you to discuss with your tax and/or financial advisor what is best for you and your financial planning and tax situation.

Matching Gifts

Increase the value of your donation to the Agnesian HealthCare Foundation by applying for a matching gift from your employer. This is a great way to provide additional support at no added cost to you. Check with your employer for details.

In-Kind Donations

Due to space and health safety restrictions, we are unable to accept gifts of plants, furniture, artwork, food, or latex balloons.
